After years providing primary care for the most vulnerable in downtown Columbus - those living with Severe Mental Illness (SMI), experiencing housing insecurity, and those with Substance Use disorder - I have decided to bring my experience and practice closer to home… Clintonville.
My business partner and I have created Form & Function Healthcare to bring exceptional care to Clintonville. We are excited about the potential to grow a clinic that focuses on the patient and community - not profits, greed, or ego.

What is Primary Care?
Primary Care is the most important part of your lifelong health experience. A family practitioner follows patients from birth to death and many will treat generations of families, providing continuity of care.
Preventative Care
Regular screenings for cancers, like breast, colon, prostate, and cervical, are completed through your PCP. Your PCP will track vaccinations to make sure you are protected according to your risk factors.
Chronic Conditions
Chronic conditions such as diabetes, high blood pressure, high cholesterol, depression and anxiety, even HIV, can all be managed through your PCP.
Common Illnesses
Colds, flus, and minor injuries are managed by your PCP. Testing for strep throat, CoVID-19, Infuenza A/B, RSV, and more can be completed at your local clinic.
Coordination of Care
Your PCP will make sure you see the correct specialist and keep your health information in one place, as well as being your advocate in a complex system.
Other Specialties:
Treating infectious disease like Hepatitis B and Hepatitis C, as well as HIV.
Creating a comprehensive plan to treat Substance Use Disorder, including alcohol and opioid use.
LGBTQ care, including gender-affirming care.
What is Functional Medicine?
Lately, a lot of attention is being focused on “Functional Medicine.” Functional medicine is a personalized, patient-centered approach to healthcare that focuses on identifying and addressing the root causes of illness, rather than simply treating symptoms. It considers the body as a complex, interconnected system and incorporates factors like genetics, lifestyle, and environment to develop individualized treatment plans.
“‘Functional medicine’ is a fancy term for what Nurse Practitioners have been doing since the beginning.
We do not just treat illness, like in traditional medical practice. Nurse Practitioners have been trained to ask ‘WHY’ the illness is happening and ‘WHAT’ we can do to address the root cause - focusing on the patient as an individual within a community in an environment.”
Only treating symptoms does not fix the problem. Nurse Practitioners have been trained to use a holistic approach, ask the right questions, and work with the patient within their comfortability to identify the root cause. Treatment strategies include lifestyle changes, dietary modifications, and other therapies with the ultimate goal to address patient needs and improve outcomes..
